It stimulates “Muscarinic cholinergic receptors” in the autonomic nervous system. This muscarinic acetylcholine receptor mediates various cellular responses, including inhibition of adenylate cyclase, breakdown of phosphoinositides and modulation of potassium channels through the action of G proteins. These receptors are found all over the body, including the muscle which surrounds the bladder and the muscles of the gastro- intestinal tract. Stimulation of the bladder muscle receptors enables the bladder to empty while stimulation of the esophageal (gullet) muscles prevents the reflux of stomach contents.

Ganglion blocking compounds may cause severe fall in B.P. Cholinergic drugs: Additive effect may occur.
Quinidine, Procainamide: May antagonize cholinergic effects of bethanechol.
